Why I built this

I'm a working therapist. The big platforms I see clients through will happily transcribe my notes for me — and I decided I didn't want them to.

I couldn't get comfortable handing my clients' most private words to a service and trusting whatever happens to them next. So I built the alternative I wanted for my own practice: a tool that runs entirely on my Mac, where the therapist owns the whole process from the first word of the recording to the note you paste into your chart. Nothing is uploaded, nothing is mined, nothing leaves the room.

That's the whole idea behind Innerbloom — your notes, your machine, your call.

Nothing leaves your Mac

The most private place for your clients' words is the one that never sends them anywhere.

There's no server, no account, no cloud. Audio is captured, transcribed, and summarized entirely on your machine — by a language model that ships inside the app. No download, no sign-in, no API key: it runs fully offline the moment you launch. You can pull the ethernet cable and watch it work.

How it works

Record

Start the session from the toolbar. Your voice and the client's are captured as two separate tracks — the client's straight from Zoom's audio, tapped natively by macOS. No BlackHole, no virtual drivers, no cables to route.

Transcribe

On-device speech recognition turns both tracks into a single, speaker-tagged transcript — who said what, in order, with overlapping speech flagged.

Note

The local model drafts your clinical note in the format you chose, then hands you a "Copy for EHR" button that clears the clipboard after 90 seconds.

Last week's themes fold into this week's note — automatically.

After each session the app pulls out goals, themes, risk history, and homework into a structured carryover you can edit. The next session's summary already knows where you left off, so continuity of care doesn't depend on your memory at 6pm.

Encrypted database

Every client, session, summary, and carryover lives in a SQLCipher-encrypted file — locked behind your passphrase.

Encrypted audio on disk

Session recordings and transcripts are AES-256 encrypted at rest — the files on disk are ciphertext, unreadable without your passphrase.

Secure deletion

Once the note is in your EHR, archive the session and securely delete the on-disk audio and transcript in one step.

Two-party consent

A consent toggle at the start of each session helps you keep documentation habits consistent.

PHI-access audit trail

A tamper-evident, hash-chained log records who accessed which record and when — with no PHI in it. Review, verify, and export it any time (§164.312(b)).
